Some venues — schools, churches, HOAs, apartment communities, and city parks — require proof of insurance before they'll let us set up. No problem! Fill out the form below and we'll get your Certificate of Insurance (COI) sent straight to your venue.

👉 Tip: Most requests just need your venue's name and address, which we've got below. If your venue specifically requires a separate Certificate Holder (like a property management company, school district, or HOA) listed as Additional Insured, check the box below and add their exact name and mailing address in the notes field.
